2009-2010 Instructions for Release Forms for Players Not Residing in the LISD Boundaries

• Forms must be received by GLASA by July 15, 2009 for the Fall 2009 season!
• Forms are required for each soccer player who does NOT reside within the LISD Boundaries.
• Argyle, Bartonville and Carrollton residents who live in the LISD boundaries DO NOT have to complete the Release process.

1. Lantana and Denton Residents (Read and follow ALL instructions!)
   Fill out the forms indicated below and submit them to the GLASA office before July 15, 2009.  GLASA will then make sure the release is obtained from Denton Soccer Association (DSA).
   
   
   1. Release Form: Fill out the top section with player information, Section 3 (Player Requesting Release from Home Association) and sign the parent signature line at the bottom.
   2. Player's state issued birth certificate or passport
      • Players new to GLASA this season must submit two (2) copies: one (1) for GLASA files and one (1) to be sent to DSA
      • If player has played last season and we have birth certificate we do not need their birth certificate again
   3. Please make check payable to DSA for $25

      Submit all of the above to the GLASA office before July 15, 2009.

2. Coppell, The Colony and Lake Cities Residents
   All forms are due by July 15, 2009.   Please note that THESE associations do not charge for Releases
   
   
   1. Release Form: Fill out the top section with player information, Section 3 (Player Requesting Release From Home Association) and sign the parent signature at bottom.
   2. Fax to your home association
      • Coppell (972) 304-8791
      • The Colony (972) 625-9912
      • Lake Cities call to set up time to get form signed (940) 497-5109.

      Once you have secured your release fax to GLASA at (972) 221-4619 with a copy of the state issued birth certificate of the player.

      Each fall NTSSA requires that a new release be done even if you’ve received a release from them previously.

   3. Coppell will then send the Release Form to GLASA without contacting you further.
      
      
3. Carrollton/Farmers Branch (CFBSA) Residents
   
   
   1. Release Form: Fill out the top section with player information, Section 3 (Player Requesting Release From Home Association) and sign the parent signature at bottom.
   2. Contact CFBSA at cfbsoccer@verizon.net or (972) 245-9307 to find out the cost and procedure to obtain a release from their soccer association.
   3. Once the release form has been signed by an agent of CFBSA, it may be faxed to GLASA at (972) 221-4619.
      
      
4. Other Non-GLASA Residents
   Contact your local soccer association or GLASA for instructions.
